The Transplant Coordinator/Nurse Practitioner responsibilities include all phases of the transplant process from initial referral through outpatient follow up. This position will be responsible for providing comprehensive assessment, diagnosis, planning, implementation, and evaluation to pediatric heart transplant candidates and recipients. The responsibilities include: Coordination of an interdisciplinary medical care delivery system under the direction of the Medical Director; Serving as a liaison between patients, family, medical staff, referring physicians, and insurance companies; Developing and providing individualized patient/family teaching; Providing ongoing assessment of appropriate nursing interventions for effective use of resources and quality assurance. Demonstrates knowledge of, and compliance with, OPTN policies and CMS regulations. Organizes and participates in quality meetings and QAPI activities. Additional responsibilities include rotational on call duties for donor/recipient heart lung transplant coordination and liaison function with organ procurement agencies; data collection and documentation for clinical management and certification requirements; developing educational/teaching materials and protocols; development of clinical protocols, policy, and procedures; community education and public relations.
