Based on the needs of the department and nature of this role, pediatric Hematology Oncology and Case Management experience is strongly desired for consideration.  

This RN Case Manager will be working with the Solid Tumor Case Manager within the Outpatient Hematology Oncology Clinic.  The RN Case Manager will be scheduled 2 days per week, sharing responsibiities with one other RN Case Manager, who will work the other 3 days of the week. 

JOB SUMMARY:

Peckham Center Case Manager coordinates provision of care across the continuum for pediatric patients with a hematology and/or oncology diagnosis. The Case Manager advocates for the patient and family through triage, education, and navigation of insurance authorization requirements, as appropriate, to the nursing role. The Case Manager identifies, accesses and allocates resources to meet the goals of care, as identified by the multidisciplinary team, in partnership with the patient and family.

MINIMUM QUALIFICATIONS:

Bachelor's Degree

3 years of experience

California RN License

BLS for Healthcare Providers, issued by AHA

Must qualify for CCS Paneling

Must have good problem-solving skills

Excellent oral and written communication skills

Able to manage large call volumes and triage patient care needs appropriately

Experience working with children and adolescents with hematology and/or oncology diagnoses

PREFERRED QUALIFICATIONS:

Master's Degree

5 years of experience

Bilingual - English/Spanish

POSITION DETAILS:

Schedule: Day, 2 shifts per week

FTE: Part-time/.4FTE (32 hours per pay period)

Location: Main Campus - Acute Care Pavilion (San Diego, CA)
