JOB SUMMARY:

The Sr Lead Care Manager (LCM) operates as a member of the Enhanced Care Management (ECM) team and is responsible to be a resource and support the team in a charge role during daily operations. They must be a subject matter expert on all aspects of the ECM program and its regulatory and quality requirements as well as any community supports. The LCM will also have an ECM patient caseload. The LCM will enroll patients in ECM, perform screenings, reinforce teaching, make referrals, and provide support to implement the Care Plan developed by the Care Management Team. They will serve as the preceptor for any new hire LCMs. Other responsibilities include chart audits to ensure documentation requirements are being met. The Sr LCM assists the team and the patients/families to develop strategies to improve health and well-being, and to navigate and access community services and other resources. This position involves community outreach; for example, home visits are required.

MINIMUM QUALIFICATIONS:

Bachelor’s Degree

1 Year of Experience

CA Driver’s License

Basic Computer skills

Good communication skills, including listening well, and using language appropriately

Ability and willingness to provide emotional support, encouragement, and motivation to patients

One year experience working as a LCM

Demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment

PREFERRED QUALIFICATIONS:

3 Years of Experience in Pediatric Healthcare

Master’s Degree

Proficient in Healthcare Quality

Successful completion of a Community Health Worker training program

Bilingual: English/Spanish

Experience working in a multi-cultural setting

Experience working in a community-based setting for at least 1 to 2 years

Knowledge of some medical terminology

The current salary range for this position is $30.96 to $42.57
