JOB SUMMARY:

The HomeCare Clinical Pharmacist coordinates the daily operations of the HomeCare pharmacy program. Provides clinical and technical support to the pharmacy technicians, HomeCare nursing staff, physicians, and patients/caregivers. Ensures safe storage, compounding, dispensing, and delivery of pharmaceuticals. Clinically monitors and maintains current patient database. Participates in HomeCare and interdepartmental Performance Improvement, Quality Control and Utilization Management projects. Participates in HomeCare and RCHC Committees, meetings and projects as appropriate; promotes a self-directed work team approach with the HomeCare pharmacy and practices clinical pharmacy in accordance with pharmacy law, accreditation standards and Title XXII.

MINIMUM QUALIFICATIONS:

Bachelor's Degree

1 Year of Experience

Current licensure as a Registered Pharmacist in California

Experience in home infusion or hospital pharmacy practice or completion of an accredited residency program

Competent in keyboarding and in utilization of various computer programs

Knowledge and experience in appropriate handling and disposal of hazardous chemicals and infectious waste

PREFERRED QUALIFICATIONS:

3 Years of Experience

Experience in home infusion or hospital pharmacy with emphasis in pediatrics

Bilingual English / Spanish

The current salary range for this position is $70.96 to $97.57
