Community Health Worker (Casual) - Managed Care Clinical Operations

JOB SUMMARY:
The Community Health Worker (CHW) will be responsible for helping patients and their families to help understand their health condition(s), reinforce teaching, and/or provide coaching to sustain teaching and Care Plan from Care Management Team, and help patients/families develop strategies to improve health and well-being; navigate and access community services and other resources; and build understanding to assist patient families to adopt healthy behaviors. The CHW supports providers and the Care Management Team through an integrated, holistic approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their family. The CHW will also provide social support and informal coaching, as well as advocate for individuals and community health needs. This position involves community outreach, such as home visits and health screenings will be required.
MINIMUM QUALIFICATIONS:
H.S. Diploma,GED,or Equivalent
CA Driver's License
Proof of Auto Insurance
Must complete formal Community Health Worker (CHW) training and obtain certification with 6 months of hire
Basic computer skills
Good communication skills, including listening well, and using language appropriately
Ability and willingness to provide emotional support, encouragement, and motivation to patients
PREFERRED QUALIFICATIONS:
Associate's Degree
2 Years of Experience
Bilingual English/Spanish
Experience working in a multi-cultural setting
Experience working in a community-based setting for at least 1 to 2 years
Knowledge of some medical terminology preferred
The current salary range for this position is $21.00 to $28.88
