JOB SUMMARY:
Case Managers provide outreach and support services to children and families receiving mental health services that need additional assistance to enhance their health, wellness, and their ability to take advantage of the treatment services offered by the mental health system. The Case Manager obtains knowledge of community resources and develops collaborative relationships with other service providers and agency contacts in the community. The Case Manager connects the patient and family to services that could benefit the families, including housing, medical needs, substance abuse treatment, legal services, emergency services, and multi-cultural services. Case managers also provide comprehensive needs assessments when referred by the child's treating therapist to determine which resources to pursue for linkage with families. Service activities include Needs Assessments, referrals for resources, linkage to resources, educating and modeling access to those services, and developing self-advocacy skills to empower families to meet their own future needs. Case managers offer rehabilitation services in the form of coaching parents to help their child meet their treatment goals and assisting children and adolescents in areas such as social skills or independent living strategies. Ongoing collaboration with treating professionals is essential and comprehensive and timely documentation of all contacts is required. Case Managers occasionally attend community health fairs, school open houses or present services to other community organizations.
MINIMUM QUALIFICATIONS:
H.S. Diploma, GED or Equivalent
1 year of experience
California Driver's License and Proof of Auto Insurance
Must have good problem-solving skills
Good oral and written communication skills
Willingness to work with over-burdened, chaotic families with low resources
Experience working with Children and Adolescents
Have own vehicle with proof of insurance
Strong communication skills
PREFERRED QUALIFICATIONS:
Bachelor's Degree
2 years of experience
Course work in the area of Child Development, Behavioral Health or related field
Experience working in the County Mental Health system
Bilingual-English/Spanish
The current salary range for this position is $24.26 to $33.36
