Responsibilities
JOB SUMMARY:
From an office setting, coordinates the delivery of intermittent home health services within a multidisciplinary team. Evaluates patient medical needs, current functional status, level of care required in the home, the physician order, and the ability of the Home Care staff and caregiver to initiate and provide safe, appropriate care in the community and home setting in cooperation with the physician, community and hospital referral sources. Functions as a pediatric home health resource and expert in all areas of nursing practice, regulatory requirements and reimbursement. Exhibits a comprehensive grasp of clinical and business situations and effectively mobilizes resources to achieve patient care, department and organizational outcomes. Able to identify limitations of self and/or other clinical staff and access additional resources as needed. Requires a commitment to patient/family, development of self and team, and the acquisition of new skills/knowledge.
MINIMUM QUALIFICATIONS:
Associate's Degree in Nursing
3 years of experience
Current California RN License
BLS for Healthcare Providers, issued by AHA
Keyboard and computer navigation proficiency
Microsoft Office and electronic EMR expertise
Knowledge of VAD care
PREFERRED QUALIFICATIONS:
Bachelor's Degree in Nursing
4 years of experience
Knowledge of VAD care, home care procedures/equipment
Bilingual English/Spanish
The current salary range for this position is $59.93 to $87.58
A candidate’s placement in the range will be determined based on the terms and conditions of the applicable collective bargaining agreement.
